offices are open for appointment scheduling,
prescription refills, clinical and administrative questions,
etc.
MONDAY– FRIDAY 9:00 AM to 5:00 PM

are required; however, same day appointments
are possible. If you have an urgent situation,
please call our office before you come.
If
you are unable to keep an appointment, please
call and let us know at least one full business
day in advance, so we may have the opportunity
to offer that appointment to another patient.
We make a sincere attempt to honor your appointment
time; however the nature of our specialty sometimes
makes delays unavoidable. |

| We participate
with most major health insurance carriers. If you
are not certain if we accept your insurance, please
call our office to confirm our participation with
your carrier.
As a service to our patients, we will file the
required insurance claims to your insurance carrier;
however, you may be financially responsible for
payment of any fees not covered by your insurance.
We recommend you become familiar with your health
insurance policy, coverage, limitations and regulations
concerning referrals and pre authorizations.

Payment
Payment is
due at the time of your visit. If we participate
in your health care plan, we will collect
your co-payment or deductible at that time. For
your convenience in paying your co-payment or deductible,
we accept
Visa and MasterCard, as well as cash and personal
checks.
